Cancellation Policy

Medical Release Statement Agreement: I hereby certify that my child has been examined recently by a physician, is physically fit and has no pre-existing condition that would prohibit participation in the strenuous physical program of the school. I authorize the faculty or its representatives to obtain emergency medical treatment for the above names student, if deemed necessary, and I agree not to hold the school, its faculty, staff or their representatives in any way liable. I have provided a list of medications taken on a regular basis and the condition for which they are being taken. Release of Liability In exchange for participation in dance classes, private lessons, and/or dance-outs organized by the Aisling Mor Academy of Irish Dance taught by the ADCRG, TCRG, and/or any approved instructor, I agree for myself and (if applicable) for the members of my family, to the following: 1. I agree to observe and obey all posted rules and warnings and further agree to follow any oral instructions or directions given by any teacher, employee, representative or agent for Aisling Mor Academy of Irish Dance. 2. I recognize that there are certain inherent risks associated with Irish step-dancing and/or exercises and I assume full responsibility for any injury to myself and (if applicable) my family members, and further release and discharge any teacher, employee, representative or agent of Aisling Mor Academy of Irish Dance, for any loss or damage arising out of my use or my family’s use of or presences upon the facilities of Aisling Mor Academy of Irish Dance, whether caused by the fault of myself, my family, or other third party. 3. I agree to indemnify and defend Aisling Mor Academy of Irish Dance against all claims, causes of action, damages, judgments, costs or expenses including attorney fees and other litigation costs, which may in any way arise from my or my family’s use of the facilities of Aisling Mor Academy of Irish Dance. 4. I agree to pay for all damages to the facilities of Aisling Mor Academy of Irish Dance caused by my or my family’s negligent, reckless, or willful actions. I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY SIGNING THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.
